Outcome
I do not uphold Mr E’s complaint and no further action is required by the Trustee or WTW.

The complaint is upheld against Aviva because it failed in its duty of care in negligence to deal with Mr N’s case before he died. It took too long to provide information to Mr N for him to have made an informed choice regarding his options before he died.
Aviva shall:-
The complaint is upheld and to put matters right Teachers' Pensions shall pay Mrs S £1,000 in recognition of the serious distress and inconvenience she has suffered.
I uphold part, but not all, of this complaint. To put matters right for the part of the complaint that I uphold, the Trustee shall pay Mr R £500 in recognition of the significant distress and inconvenience caused.
Mr N’s complaint against TP is upheld in part. To put matters right, TP shall follow the recommendations in paragraph 70 below.
Mr N has complained that TP has:

Mrs E’s complaint is upheld and, to put matters right, ReAssure shall reconsider its decision about the distribution of death benefits under the Scheme in accordance with the New Rules.
ReAssure shall also pay Mrs E £1,000 for the serious distress and inconvenience caused to her.
